Just installed UserForms 0.4-122 on a SS 3.0 site. The readme indicates the version requirement to be "SilverStripe 3.0"
Copied module to site root /public_html/userforms
Ran /dev/build
Created a new page as "User Defined Form" and although I see "Form", "Form Options" and "Form Submission" tabs, the "Form" tab is blank?

0.4 is for 2.4
I've got the same problem (the 'form' tab is blank on a SS 3.0 site).
I installed it via composer using the following line:
"silverstripe/userforms": "1.0.*@dev"
so, so far as I'm aware (I'm new to composer), this should be version 1.0?
Same problem here with silverstripe 3.1 head and userforms head. Everything else works smooth so far.
But the "Form" tab in the admin panel remains empty.
I had the same problem: SS3.1 with the latest master version of Userforms from Github (today) and the Form tab was showing up blank.
I did a flush on the actual page in the CMS and it returned. eg. /admin/pages/edit/show/3?flush=1
A normal flush on /admin wasn't enough, it had to be on the actual/specific page.
