Add- on userforms - How add the fields ?

I use my PC then I installed WAMP 2.2 Siverstripe version 3.1.2 then I downloaded and install add-on userforms compatible with SilverStripe> = 3.1, then I created a new type of page userform but I can not find the section where you can add the fields of form.
Probably something wrong.
Can you help?

Make sure you flush the CMS - http://localhost/SilverStripe/admin/pages/edit/show/6?flush=1
