Hmm...
1) It might take some time for the button to show up.
2) Have you been extending the module? In that case you most likely made a slight coding-mistake (fx forgot a ")" or something like that)
Hmm... I've only heard of this happening in Linux systems. Try this.. If you're in Firefox, right click on the popup window and select "View frame in new tab"
Tell me if it shows up.
