After a clean install of Silverstripe using composer on a Windows machine with XAMPP, I get this error when trying to add a new page "User defined form". I have attached the file in question (rename to .php).
One thing worth mentioning, is that while installing SilverStripe Apache crashed numerous times. This happened in a VM, too.
Any help is appreciated.
