Newsletter module static subscription form

Hello

I've just downloaded and installed current trunk version of newsletter module. Main problem for me now is how to build form for users subscriptions/unsubscriptions like in attachment ( Ajax prefered ). I've red somewhere that module now use UserForms module to provide such subscription form but I'd like to define such one directly in code.
